Hi! My name is Sara. I have been a private practice IBCLC since 2018 and a nurse since 2010. I've always been a bit particular about my charting... and also about how to load the dishwasher, and what tasks need to be completed before bedtime, and the organization of  every. single. item in my home... you get the idea! In my opinion, one of the best private practice charting platforms is IntakeQ. One of the things that I love most about IntakeQ is the customization that is possible in so many  areas- especially in the charting. In the past couple of years I have seen numerous social posts, and been involved in numerous conversations about the tedious task of setting up charting. I began to have colleagues ask me to share my IntakeQ forms- and since I am all about all of us supporting each other, I have. They have been a hit, so here we are. The intake form conveniently directs interested families to check for coverage with both TLN and Wildflower, as applicable and if desired. Language is gender neutral and family dynamic inclusive.
